This patient presented with a bulbous nose appearance, characterized by a rounded and widened nasal tip, along with insufficient projection and definition of both the bridge and tip. As a result, the nose appeared somewhat “buried” within the face, lacking central facial definition and overall balance.

In cases like this, simply making the nose higher is often not enough.

To achieve a more natural and refined result, it is important to consider not only the bridge height, but also:

• The width and direction of the nasal tip
• The connection between the bridge and the tip
• The bulky soft tissue volume commonly seen in bulbous noses
• The overall facial definition created by the nose

Recently, many patients have been requesting what is commonly referred to in Korea as a “natural glam nose line.”

However, many people misunderstand this concept as simply creating a nose that is glamorous yet natural-looking.

In reality, if facial definition is lacking and the tip is made excessively high or dramatic without considering facial balance, the nose may appear unnaturally projected or disconnected from the rest of the face.

For this patient, rather than pursuing excessive height, the surgical plan focused on allowing the nose to create natural central definition and harmony within the face.

Surgical Planning & Technique

The nasal bridge was enhanced using a silicone implant designed at a proportionate height based on the patient’s facial balance, helping restore central facial definition while maintaining a natural appearance.

The nasal tip was refined using:

• Tip cartilage repositioning
• Tip cartilage suturing
• Soft tissue debulking
• Structural tip support using autologous cartilage grafting

These techniques helped reduce the bulky appearance of the bulbous tip while creating a slimmer, more refined, and structurally balanced nasal shape.

A structural support graft was placed between the tip cartilages to help prevent long-term loss of projection or tip spreading over time.

In bulbous nose rhinoplasty, simply narrowing the width alone is not enough.

It is important to create a natural flow between the bridge and tip while refining the central structure of the nasal tip itself.

In many bulbous nose cases, the skin envelope around the tip is also relatively thick.

For this reason, beyond simple tip suturing techniques, careful soft tissue debulking and strong structural support are often necessary to create a sharper, firmer, and more refined tip contour.

As the tip structure becomes more defined, the overall width of the nose can appear naturally reduced, creating a more sophisticated facial impression.

Natural Glam Rhinoplasty & Facial Definition

Modern Korean rhinoplasty trends are evolving beyond simply creating results that look “natural” or unnoticeable.

Today, many patients prefer a refined “natural glam” nose line that maintains harmony while still adding elegant definition and subtle sophistication.

Especially in patients whose noses appear visually buried within fuller cheeks or whose faces appear flatter or heavier due to weak nasal definition, improving central facial projection alone can significantly change the overall impression of the face.

For this patient, the goal was to preserve the patient’s naturally soft and balanced appearance while enhancing facial definition and creating a more refined and sophisticated overall impression.

The columella, nasolabial angle, and philtrum line were also carefully adjusted to create a smoother and more natural transition from the nose to the lips.

As a result, the profile line appears more balanced and refined, rather than simply looking like the nose was made higher alone.
